Please use this form to submit your items for this summer’s Garage Sale.

You can choose three different kinds of payment:
1. Cash payout for 50% of sale price (excluding sales tax)

2. Store credit payout for 75% of sale price (excluding sales tax)

3. Donate all proceeds to Extra Life, benefiting Valley Children’s Hospital

If you’d like different kinds of payouts on different items, please fill out a form for each kind (there can be up to 10 items on each form)
